As PCN practices we work with others to expand our capacity and the care we can provide. We have a fabulous team of people working with us who we would like to introduce here
First Contact Practitioners
Working within the surgeries as a first port of call First Contact Practitioners can provide assessments and care for people with conditions that sit within their area of expertise.
Appointments can be booked through the surgery.
- Anthony Grinstead: Physiotherapist (based at Ball Tree Practice & Orchard Surgery)
- Will King: Physiotherapist (based at New Pond Row Practice)
- Jack Wakelin: Paramedic
While the physiotherapists can give their expert assessment and advice about musculoskeletal conditions, they do not provide physiotherapy in the surgery, but can they refer on for this should it be needed.
You can also self-refer for physiotherapy via the University Hospitals Sussex website
Jack, our Paramedic is also able to visit along with Ball Tree Practice’s Advanced Nurse Practitioners and New Pond Row’s Paramedic.
Medicines Management Team
We are really lucky to have an extensive ‘Medicines Management Team’ that work within all 3 surgeries. They can answer many of the medicines queries that previously may have gone to your GP or a Nurse, and they all work closely together to ensure we are safely supporting your care.
Pharmacists
Alongside managing much of our day to day ‘Medicines Management’ and ‘Prescriptions’, our experienced Pharmacists support with monitoring the quality and safety of our prescribing, including reviewing people’s medication with them.
They are also able to support with Hypertension (Blood Pressure) and Cholesterol Treatment and ‘Lifestyle Advice’ looking at ‘Cardiovascular Disease’ treatment and risk reduction.

Pharmacy Technicians
This team works alongside the Pharmacists supporting with the day to day medication management along with improving safety and quality and answering the daily queries.

Mental Health Support
We work with ‘MIND’ to provide ‘Mental Health Wellbeing Social Prescribers’.
They can explore ways to manage your symptoms as well as considering your emotional wellbeing as a whole including exploring the external pressures that may be affecting mental health e.g. Finance, School or Work pressures.
This can be done over up to six sessions, occasionally more if needed.
A referral is made through your surgery.

- Rosie Elliot: Clinical Mental Health Practitioner (Supports to compliment this team and provide additional care, workshops and links to the Adult Mental Health Team).
Children's Mental Health
- Diana Stevenson: Children 12-18 Years Emotional Wellbeing Service
- Denisa Stoean: Children 8-11 Years Emotional Wellbeing Service
Patient Care Coordinators
Patient Care Coordinators are based in each surgery to provide support and signposting for those in need.
They coordinate the care for certain groups of people including those who are housebound, frail, have dementia or mental illness and who are carers. Please see their dedicated page.

Social Prescribers
By working with the ‘Going Local’ Service we are able to provide ‘Support’ and ‘Wellbeing’ in many areas of need including ‘Connecting in the Community’, ‘Isolation’, ‘Housing Issues’, ‘Financial Issues’ and ‘Information Technology Skills’.
You can be referred from the surgery, or self-refer from their website
It may be a just a couple of telephone calls and signposting on to other services, or you may benefit from a series of more in-depth sessions to explore what you may need.
